How can I donate an item to my loved ones prison?

To make a donation to your loved ones prison for things like fans, books, cards, etc. follow these easy steps.

1. Download this FDC Donation Form

2. Complete the Donation Form and sign it.

3. Email the signed form to the Assistant Warden of Programs at your loved ones facility.

4. Once you receive an email back with the donation form signed by the Assistant Warden or Warden, print a copy and have the item shipped to the institution or drop the same off on any visitation day at 8 am along with a copy of the donation form signed by both you and the facility.
